TERMS OF SERVICE
NO-SHOW/LATE CANCELLATION POLICY
We understand that you may need to cancel an appointment occasionally. In such circumstances, please contact us no later than 24 hours before your scheduled appointment time. You may do so by calling 480-904-4439, emailing gina@tomewellnessaz.com, or using the patient portal.
If you do not show up for your appointment or cancel or reschedule within 24 hours of your appointment time, we will consider that a no-show. No-show appointments are subject to a $50 fee. No-show fees are the patient’s sole responsibility and due prior to your next appointment. If the no-show fee might prevent you from receiving necessary care, please contact us.
We know that unexpected situations sometimes arise. In the case of emergencies or extenuating circumstances, the no-show fee may be waived. Waivers are determined on a case-by-case basis at the practice management's sole discretion.

APPOINTMENTS
To ensure that each patient is given the proper amount of time allotted for their visit and to provide the highest quality care, it is important for each scheduled patient to arrive for their visit on time. If it is necessary to reschedule an appointment, please call us immediately.
Reminder texts and emails are sent 3 days prior to your appointment. These are COURTESY notifications.
Patients arriving late for an appointment by 15 minutes or more may need to reschedule.
CANCELLATIONS
We ask that you provide a minimum of 24 hours’ notice if you are unable to keep your appointment. Less than 24 hours’ notice of cancellation will be considered a “Missed Appointment” or “No-Show”.
TELEMEDICINE
As a courtesy Tome’ Wellness + Aesthetics offers telemedicine visits. Telemedicine visit cost is $125.00. We require a reservation fee of $50.00 to hold your appointment. This will be applied to your office visit charge. However, if you fail to keep your appointment and/or do not cancel 24 hours prior, you will forfeit the reservation fee of $50.00.
